Consultant – Preparation of Urban Health Promotion Center (UHPC) Operational Protocol 

Context

Médecins du Monde France (MdM-F) has been operating in Nepal since 1995. From January 2018 to December 2019, MdM-F implemented a 2-years program titled “Healthy Waste Workers in Kathmandu Valley” with an overall objective to improve the health status of informal waste workers in Kathmandu Valley. After Nepal entered the first official lockdown from 23 March 2020 due to COVID-19 pandemic, MdM-F developed a short 4 months (June 2020 to September 2020) response project aiming to mitigate the spread of the COVID-19 amongst the vulnerable communities of formal and informal waste workers in Kathmandu Valley / Kakani Rural Municipality and amongst the urban community preventing and promoting safe and efficient waste collection, in partnership with Kathmandu Metropolitan city (KMC), Solid Waste Management Association (SWMAN) and UNDP.In continuation with the previous 2018-2019 project, MdM-F has been implementing a 3-year project (2020-2023), named “Healthy Waste Workers for Sustainable Waste Management” (HWW4SWM) which overall objective is to improve health, wellbeing and livelihood of informal waste workers in Kathmandu Valley and Sisdole landfill. 

This project has three main expected outcomes:

  1. Formalization of IWWs through non-profit enterprises and increasing their recognition in integrated solid waste management in urban cities
  2. Increase risk mitigation and promotion of decent safe work opportunities in current activities and through innovation 
  3. Increase access to quality health services for IWWs and the community through promotion of Urban Health Promotion Centre

Outcome 3 of the project is related with the medical component and will be implemented in 2 areas: a dense ward of Kathmandu City (Ward 19) and a rural ward of Kakani Municipality (close to Sisdole landfill), with 2 outputs: 

  • To support the local health system in transitioning from an urban health clinic (UHC) at ward 19, Kathmandu to an urban health promotion centre (UHPC). 
  • To support the availability and use of quality health services including occupational health service in supported health facility (HF) for the IWWs and community. 

MdM is supporting Kakani / Sisdole health facility since 2018 but the development of this component in Kathmandu Ward 19 is a new activity. 

The HWW4SWM program has been set up in partnership with 4 local stakeholders:  

  • Kathmandu Metropolitan City, responsible of urban health facilities 
  • Kakani Municipality, responsible of the Sisdole health facility 
  • SamyuktaSafai Jagaran(SASAJA), an IWWs’ cooperative experienced in IWWs mobilization, peer education and microfinance 
  • Green Path Nepal, specialized in developing innovative waste management projects based on public-private partnerships, including harmful and toxic waste management.

Key objectives of the consultancy

These terms of reference concern a preparation of UHPC operational protocol which is aims to:

  • Prepare UHPC Operation Protocol/manual (covering all aspects of UHPC management and monitoring, service provisions at clinic and community level),
  • To make common understanding on roles and responsibilities for smooth functioning of all UHPC services,
  • To finalize relevant tools and guidelines required for the operation.

Methodology for the preparation of UHPC Operation Protocol 

  • Collect relevant document for protocol preparation, desk review, relevant article, government guidelines and policy paper related to UHPC,
  • Visit/zoom/telephone meetings with UHPC staff and local stakeholders, potential partners to understand the current situation,
  • Prepare outline of the documents and finalise in coordination with MDM team including MDM Medical Referent from MdM HQ in France
  • The document should contain broadly the following: 
    • Program Policies and Parameters
    • Training, Communication and Outreach
    • UHPC setup, temporary isolation and referral of cases
    • Patient enrolment
    • Patient consultation
    • Service Provision at the UHPC
    • Laboratory Management and services
    • Pharmacy Management and services
    • Follow up of patients
    • Data Management/ Collection
    • Monitoring and Evaluation
    • Stakeholders roles and responsibilities
  • Propose, design and schedule workshops
    • Initial Workshop: Need based initial workshops with UHPC team (including support staff), Ward Management Team/Health team, KMC relevant personnel, Local partner, community representative (preferably FCHV and social mobilisers) and MDM team. The workshop would discuss and agree on the contents of the documents. 
    • Final Workshop: Two-days’ workshop with the same participant as described above for inputs and finalisation on the document.
  • Write up of comprehensive documents including all the annexes
  • Field visit and Focus Group Discussion on the drafted document.
  • If needed present summary of protocol document to MDM relevant staff after incorporation of feedback from Final Workshop.
  • Finalize and submit the document package including all tools/formats - after final feedback from the MDM team.

MdM Nepalteam (Especially Med Advisor-Nepal, Program Manager and Med referent from HQ) will be involved in different meetings, UHPC staff workshop and presentation of protocol summary as per relevancy

Expected deliverables and presentation findings

The consulting form will provide a UHPC protocol including: 

  • Meeting minutes of briefing meetings with UHPC and KMC focal person.  
  • Meeting minutes of meeting with MDM- France and Nepal Medical and Health Advisors
  • Initial Workshop and final workshop conducted with UPHC and relevant stakeholders 
  • Field visit report  
  • Final draft protocol document 
  • Final document package including protocols, updated JD’s, organogram, follow-up tool and action plan should be submitted to MDM Nepal for validation.

Organization of the protocol preparation

The field visit and office work will be implemented according to MdM and national regulations, security process and SOPs related with the context in Nepal and the COVID-19 pandemic. All activities and schedules might be adjusted depending on the local situation.

All field visits, meeting will be comprehended with MdM medical advisor. The consultant will be invited to work from home or field and to join MdM office (Kupandole, Lalitpur) for specific meetings or collective work activities. Online Skype or Zoom calls will be organized according to the local situation.

COVID19SOP to be followed during all the protocol preparation period

Note: SASAJA will cover the dissemination expenses all other costs will be included in consultancy fee.

Security / COVID-19

Briefing to be realized the 1st day and as per need, including medical briefing if necessary and presentation of SOPs. 

Required Competencies

The qualifications and/or specialized knowledge/experience required are:

  • Expertise of Consulting Firm should have senior health experts with 10 years of experience in similar work and preferably an experience in Urban Health
  • Proven experience in preparing and designing health related guideline, protocol and monitoring tools
  • Specific experience in the areas of public health
  • Experiences on coordination with government health authorities and urban health. Preferably previous experience of direct work with Municipalities and ward health team 
  • A strong commitment in delivering timely and high-quality results. 
  • Good interpersonal and communication skills in English, and an ability to express ideas and concepts concisely and clearly to ensure that the UHPC operational protocol is understood
