INVITATION TO QUOTATION

FOR AGREEMENT OF INSURANCE SERVICE

Dear Sir/Madam, 

Further to your enquiry regarding the publication of the above-mentioned invitation to Quotation, please find enclosed the following documents, which constitute the Quotation dossier:

A – Instructions to Bidders

B – Annex 2: Quotation Submission form (to be completed by the Bidders)

In order to enable the bidder to complete the forms (Quotation submission form), an electronic copy of these documents can be sent upon request. It is strictly forbidden to make alterations in the in the printed text. The bidder will be bound to the original text in accordance with the document forwarded in PDF format or by letter.   

A. INSTRUCTIONS TO BIDDERS

In submitting a Quotation, the bidder confirms that they have read and agreed to comply with the requirements, terms and conditions specified in this RFQ. The bidder must submit all required information and documents within the specified deadline. Quotations that do not meet the required conditions or contain significant deviations may be rejected.

A.1. Scope of services:

The subject of the contract is the supply of the following services by the service provider: 

• Medical insurance 
• Group Accidental Insurance

A.2. Planned time table:

The Contracting Authority reserves the right to alter the dates and time in the following timetable, in which case all bidders will be informed in writing and a new timetable will be provided.

A.3. Exclusion from award of contracts 

Contracts may not be awarded to bidders who, during the procurement procedure:

(a) are subject to conflict of interest

(b) Are guilty of misrepresentation in supplying the information required as a condition of participation and eligibility in the Quotation procedure or fail to supply this information.

(c) Perform, condone or tolerate corrupt, fraudulent, collusive or coercive practices, regardless of whether such practices can be attributed to this Quotation procedure;

(d) Attempt to influence the evaluation committee in the process of examination, clarification, evaluation and comparison of Quotations, to obtain information on how the procedure is progressing or to influence the Contracting Authority in its decision concerning the award of the contract.

A.4. Eligibility Criteria (Minimum Requirements) & Evaluation Criteria

The candidates must provide the documents listed below. Candidates that do not provide the documents listed below will not be qualified for the evaluation.

a) Company registration

b) VAT/PAN registration

c) Tax clearance certificate FY 2081/82 (latest)

Evaluation Criteria

Quotations will be evaluated based on premium, coverage, reimbursement terms, claim settlement process, and overall suitability of the proposed insurance services

A.5. Submission of Quotations and closing date:

Sealed quotations must be submitted to PHASE Nepal, Dadhikot, Bhaktapur by 29 September 2026, 5:00 PM. Quotations received after the deadline will not be considered

A.6. Services Terms:

PHASE Nepal intends to enter into a contract with the selected Insurance Company for a period of 12 months, with the possibility of extending the contract for an additional 12 months, subject to mutual agreement. 

A.7. Cancellation for convenience:

The Contracting authority may for its own convenience and without charge or liability cancel the Quotation process at any stage.
