Request for Quotation for Staff General Health Checkup

Heifer invites competitive quotations from eligible healthcare service providers and hospitals for providing general health checkup services.

Interested providers must meet the following minimum criteria:

• At least two (2) years of experience in providing similar healthcare services
• Prior experience working with International/National NGOs (I/NGOs)

Please submit your quotation along with the below documents to Procurement-NP@heifer.org mentioning email subject line “Quotation for general health checkup” by 31st March 2026, 5:00 PM. Incomplete application and applications received after closing date will not be considered.

3. Company registration certificate
4. VAT certificate
5. Tax clearance certificate FY 081-82
6. Brief profile

Evaluation Criteria:

The quotations will be evaluated based on a Quality and Cost-Based Selection (QCBS) approach, 70% quality and 30% cost.
