REQUEST FOR QUOTATION (RFQ) FOR GENE SEQUENCING REAGENTS/CONSUMABLES AND PLASTIC SHREDDER

FHI 360, an international NGO, requests for quotation from locally registered agencies to supply the following:

Detailed specifications of the above-mentioned items can be obtained via email request to atamang@fhi360.org The following documents should be submitted along with the quotation:

• A copy of PAN/VAT registration certificate
• A copy of Company registration certificate
• Latest Tax clearance certificate
• Company Profile
• List of Clients
• Compliance certificates as mentioned in the detailed specifications

The quotation should be valid for at least 90 days and must provide information on delivery time, warranty period & after sales service (for applicable items) and product shelf-life(for items containing expiry dates). Final selection will be based on favorable cost & services, delivery time/capability and fulfillment of minimum required product shelf-life for items containing expiry dates. Preference will be given to agencies who has the required items in stock or will take less delivery time.

Hard copy of the quotation along with specified supporting documents requested can be submitted in a sealed envelope clearly marked “Quotation for Gene Sequencing Reagents/Consumables and Plastic Shredder” at the following address:
